Mountain Pacific’s case management team works to support Medicaid members to improve the prevention, screening, diagnosing and management of acute and chronic diseases, including those related to behavioral health. Our case managers coordinate care between multiple health care providers, connect Medicaid members to the resources they need and provide those members individualized education about their health. We combine physical and behavioral health interventions to achieve holistic care for each person. The program also supports members’ compliance with national preventive medicine recommendations.
