Advancing Tribal Health

American Indian and Alaska Native (AIAN) populations hold the answers to improving health and health care in Tribal communities. For many years, these communities have experienced inequities in funding, resources and structural support, negatively impacting health outcomes. We work alongside Tribes and their health care communities to co-create projects that focus on patient safety, access to care, behavioral health, public health emergencies and engage patients, families and Tribal leaders in the planning, implementation and support of health improvement projects.

An important part of these efforts is the inclusion of Tribe-specific traditions and heritages to provide whole-person, culturally appropriate care that respects individuals’ wishes and health goals.
Strategic Health Planning
Mountain Pacific works directly with Tribal community leaders to support culturally informed, long-term health planning and programming. We gather a variety of data, provide community assessments and offer recommendations to enhance and support existing programs that impact the community’s health and well-being.

Chronic Disease Prevention & Self Management

Mountain Pacific takes a collaborative, community-based approach to supporting the prevention and improved self-management of chronic diseases. We provide project management services, data analysis and reporting, technology assistance and other tools, resources and process support to help health care providers and their patients to improve outcomes for those living with chronic disease and reduce the overall cost of health care.
Chronic diseases such as heart disease, diabetes, chronic obstructive pulmonary disease (COPD) and other conditions are often preventable – but if not preventable, they are manageable, especially when people who live with these diseases take an active role in their health.

The Health Insurance Portability and Accountability Act (HIPAA) regulations require health care providers, payers, clearing houses, their business associates and subcontractors to conduct a security risk analysis (SRA) and form a risk management plan.

e-Clinical Quality Improvement

Mountain Pacific developed an eCQI toolkit to employ aspects of an agile delivery cycle, which helps health care professionals and quality improvement managers achieve high-value improvements quickly and efficiently, one small change at a time. These systematic improvement cycles are called “sprints.” Each sprint provides value-added results for organizations about every two to six weeks based on the plan-do-study-act (PDSA) quality improvement cycle.
