Managing Department of Corrections Population Health

Mountain Pacific supports the Montana Department of Corrections by reviewing Hepatitis C cases among incarcerated individuals to make sure they are up to date on their Hepatitis A and B immunizations and needed lab tests. We work with health care providers serving the prison population to help assess which Hepatitis C treatment would be best for the patient by applying established guidelines and reviewing the patient’s treatment history, medications and other health issues, including liver fibrosis, which can be caused by Hepatitis C.
While about 2% of the U.S. population have the Hepatitis C virus, the percentage of those in prison with the virus can be as high as 40%.
Treating Hepatitis C in prisoners reduces hefty health care costs, decreases the risk of transmission to others when the patient is released from prison and helps stop the virus from spreading.
Mountain Pacific can work with other state and federal departments of corrections and prison systems to provide pharmacy consultations for disease management, including chronic obstructive pulmonary disorder (COPD), asthma, dyslipidemia, hypertension, type 2 diabetes, antipsychotic metabolic monitoring and anticoagulation. Our clinical pharmacists are certified in supporting the management of these chronic conditions. We offer drug utilization review to assess the safety, effectiveness and indication of each reviewed medication. We can provide diagnoses and labs for:
Drug-to-drug interactions
Drug-disease state contraindications (situations where a certain treatment may be harmful to the patient)
Drug-patient precautions for age-appropriate dosing
Drug-allergy review
We also can assess for the inappropriate use of drug treatments, including medications, supplements and over-the-counter products and make sure treatments align with recommendations from the U.S. Food and Drug Administration.
